Creating an account

In order to use the LifeSign MD questionnaires and educational modules, you’ll have to register by creating an account. You’ll be asked to provide some basic contact information and create a user name and password.

• On the home page click the Need an account? link in the Health Professionals login area.

• Enter your contact information (You don’t have to fill out all the fields, but the Physician / Practice Name and Email for Notification are required)
• Create a login name. It must be at least 6 characters long.
• Create a password (also 6 characters long). Enter it again just to confirm the spelling.
• Click Submit

You now have an account with LifeSign MD and are logged in.
