Printing Reports

LifeSign MD creates reports containing a summary of the patient’s responses to the questionnaire items (the Physician Report) and summary of the patient’s risk factors and strategies to reduce risk based on the patient’s questionnaire responses (the Patient’s Report). These can be printed out for your own review and for you to review together with your patient. To print the reports:

• Click on the Patients’ Data link
• Click on the Reports tab
• Select a patient and, if the patient has filled out the questionnaire more than once, the version you want to view.

• Click the Printable View link.
  

• A new browser window will appear. Click on the Print link.
  

You can print other pages by simply clicking on the printer icon in your browser.
