Viewing a questionnaire
You can see a questionnaire the way a patient would see it by signing in as a patient.
If you are currently logged in to LifeSign MD, you’ll have to log out first.
- Sign in to the Patients login area by typing “demo” in the Login
Name and Password fields.
- Click Login
- You will first see an overview and instructions on using the questionnaire.
- Click the Begin Questionnaire button to see the first question.
- You’ll have to actually provide answers in order to advance through the questionnaire;
you won’t be able to simply hit Next to go through the pages without
answering. However, because you signed in as “demo” your answers will
not be saved or recorded.
