Viewing questionnaire responses
LifeSign MD provides several ways to view questionnaire responses. For an individual
patient, you can view the responses as raw data or in a report. The raw data view
includes a list of all possible questions in the questionnaire, all possible answers,
and which answer the patient selected. The report view lists only the questions
the patient answered and the patient’s response.
Reports view can be printed out for your records or to be used at the patient’s
next appointment.
Viewing Patient’s Raw Data
To view a patient’s data (raw data view):
- Click on the Patients’ Data link.
- Click on the drop down box to select a patient by his/her login number and password.

- If the patient has filled out the questionnaire more than once, click on the drop
down box to select which one you want to view.
- The raw data view comes up by default.

Viewing Patient's Report
To view a patient’s data (reports view):
- On the Patients’ Data page, select a patient by his/her login number, then
click on the Reports tab

- If you want to print this patient’s report, click the Printable View
link.

- A new browser window will appear. Click on the Print link.

Two reports are printed out: a Physicians Report that groups the responses into
the following topics: Patient Questions, STI Diagnoses, Partners and Patient Perception
of Risk; and a Patient’s Report with fact sheets that address the different
methods of virus transmission — through injection drug use, sexual contact
and sexually transmitted infections — and resources where the patient can
go to get more information.
Viewing All Responses
There are two other methods available for viewing questionnaire responses. You can
view aggregate data for your patients and for everyone who has answered the questionnaire
using the LifeSign MD system.
To view aggregate data:

- Your patients’ aggregate data are listed under Your Stats
- Data for all questionnaire respondents are listed under LifeSign MD Stats

The responses are listed as frequencies (139/208) and percentages (67%)
